The $3 Billion Problem No One Talks About

Every year, Americans forfeit over $3 billion in FSA money back to their employers. Not because they didn't need healthcare. Because they ran out of time.

Your FSA (Flexible Spending Account) is use-it-or-lose-it by IRS rule. Money you put in on January 1st that isn't spent by your plan-year deadline vanishes. Your employer keeps it. You get nothing.

This guide is about not being in that group.

How FSA Expiration Actually Works

First, some mechanics most people get wrong.

Your plan year ≠ the calendar year (always). Some employers run January–December. Others run October–September. Know your plan year end date — it's in your benefits enrollment docs.

Grace period vs. run-out period:

If your plan has none of these, every dollar unspent at year-end is gone.

The Prioritized Spending List

Here's how to clear your FSA balance by dollar impact, fastest first.

Tier 1: Big-Ticket Items (Clear $200–$1,000+ in a Single Purchase)

Prescription eyeglasses or sunglasses with prescription lenses Cost: $150–$600. FSA eligible. Order from Warby Parker, Zenni, or your local optometrist. If you have any prescription at all, this is the fastest way to spend a large balance.

Contact lenses (annual supply) Cost: $200–$400 for a year's supply. Fully FSA eligible. Order through 1-800 Contacts, Coastal, or your eye doctor.

Dental work Cost: $200–$2,000+. Fillings, crowns, root canals, night guards, orthodontics (sometimes), dental bridges. Call your dentist today and ask what they can schedule before your deadline. Cleanings are usually covered by dental insurance separately — think bigger.

Hearing aids Cost: $1,000–$5,000. Fully FSA eligible. If you or a family member has been putting this off, now is the time.

LASIK eye surgery Cost: $2,000–$3,000. FSA eligible. Many providers offer payment plans + FSA for the remainder.

Tier 2: Medium Purchases ($50–$200 each)

Physical therapy sessions Your PT likely bills FSA directly. Schedule a session block before your deadline.

Chiropractic care FSA eligible. If you've been dealing with back pain, this is covered.

Mental health therapy Session copays and full session fees are FSA eligible. Schedule a block of sessions.

Prescription refills (90-day supply) Call your doctor and ask for a 90-day prescription fill for every medication you take. Then fill them — FSA eligible.

Blood pressure monitors, glucose monitors, pulse oximeters All FSA eligible medical equipment. Useful to own and easy to order.

First aid kits, thermometers, heating pads Boring but legitimate. A good first aid kit is $40–$80 and lasts years.

Tier 3: Over-the-Counter Items (Use Up Small Balances)

Since 2020, hundreds of OTC items became FSA eligible without a prescription. Use these to clear small remaining balances:

Where to buy: Amazon HSA/FSA store, Walmart FSA store, CVS, Walgreens, or any pharmacy — just pay with your FSA card.

The 3-Week FSA Clearance Plan

Week 1: Big items Schedule the dental appointment, eye exam, or PT session that requires an appointment. Get it on the calendar now — these book out fast.

Week 2: Order everything you can online Amazon, 1-800 Contacts, Warby Parker. Most FSA-eligible items ship in 2–5 days. Pay with your FSA card.

Week 3: Clear the remainder with OTC purchases Walk through CVS or hit the Amazon FSA store. Allergy meds, pain relievers, first aid supplies.

What To Do If You're Running a Large Balance ($500+)

Don't panic-buy random things. Prioritize like this:

  1. Glasses or contacts — If anyone in your family wears them, buy the best you'd buy anyway
  2. Dental — Call and ask for available appointments before your deadline
  3. PT/chiropractic — Pre-pay for a session package if the provider allows it
  4. Medical equipment — Blood pressure monitor, glucose meter, etc.
  5. 90-day prescription refills — For every medication you take
  6. OTC sweep — Allergy, pain relief, cold medicine to finish off the balance
